Web-based match-making platform for hospital discharge and transfer management

Nubedian presents a pioneering solution for hospital discharge and transfer management. Our web-based matching platform makes it possible to find suitable service providers for patients at the touch of a button, based on their profile. Through automatic data transfers between hospital information systems and service provider systems via interfaces and innovative encryption methods, we guarantee data exchange in compliance with data protection regulations and across all sectors.

How would you explain your solution in three sentences to a professional colleague from the health sector?

"Caseform is a web-based match-making platform for hospital discharge and transfer management. Based on a patient profile, suitable service providers are automatically found for patients at the touch of a button. Data is automatically transferred from hospital information systems and transferred to service provider systems via interfaces. Innovative encryption methods enable data protection-compliant and cross-sector exchange."

What problem motivated you to start the company?

"I first came into contact with (non-)digital healthcare during my civilian service in a hospital. As part of my master's thesis, I got to know the processes in the field of nursing advice and, together with my co-founder Dr. Bruno Rosales Saurer, immediately recognized the potential for web-based software solutions in the healthcare sector. I moved on from nursing advice to social counseling in the hospital context and thus to the field of discharge management. The idea of automating discharge management arose while supporting employees in discharge management who spend a large part of their working time on the phone or in front of a fax machine."

How do you convince a pharmaceutical company, a health insurance company or another potential client to set up a pilot project with you?

"Hospitals struggle with numerous challenges in the context of discharge management,

• such as a lack of evidence for auditing bodies such as the Medical Service,

• a reduction in revenue due to time-consuming work processes,

• analog technologies such as fax machines do not meet data protection requirements,

• difficulties in communication between the individual sectors and

• delayed discharges and thus longer lengths of stay.

With the Caseform discharge platform, hospitals can efficiently manage their discharge and transfer processes. The platform automatically generates service records for each search, digitizes analogue, repetitive work steps and, as a communication platform, offers a powerful tool for smooth collaboration between all stakeholders involved. This not only saves hospitals time and money, but also improves the quality of care for their patients. With nubedian GmbH, hospitals have a partner who knows the discharge processes from admission and, in addition to transfer, also offers structured documentation of patient consultations in order to meet the patient's right to choose. Our focus is on the users, who are actively involved in the development process at regular meetings."

Who are your current customer (groups) and who do you want your potential customers to be?

"Caseform already connects acute care hospitals with over 10,000 service providers for the optimal care of patients. The database includes providers of outpatient and inpatient care, medical aids and homecare, medical rehabilitation clinics and cost units. The database covers the whole of Germany. The next step will be to integrate care support centers and providers of medical aids. The transferability of the solution to Switzerland, Austria and England is currently being examined."

Where do you see yourself in 3 years and how can 5-HT support you?

"In three years' time, the Caseform discharge platform will be one of the largest marketplaces for healthcare services in Germany, with the platform also being opened up to private individuals and thus helping all those affected to find the right healthcare. 5-HT connects us with other start-ups, creates access to investors and offers visibility and reach through various event formats."

You participated in the WE CARE event in November 2023. What is your impression of the event and what have you gained (so far) from participating?

"The event was very well organized and created a  informal setting for exchange. Above all, the event gave us reach and contacts to various other start-ups that were represented at the event. As a finalist, we really enjoyed presenting our solution to the top-class jury as part of a pitch. We would be delighted to take part again, should there be a new edition, and to present our Caseform solution for digital discharge and transfer management to even more hospitals."
